Telephone Service Requests (TSRs)

This page contains information about Telephone Service Requests (TSRs).

Telephone Service Requests (TSRs) are used to order services from CITES Service Request Management. Only Telecommunications Unit Coordinators are authorized to submit TSRs. Please work with your department's coordinator if you would like to request a service. To find out who your unit coordinator is, contact Service Request Management at 333-1161.

Telephone Service Requests on the Web

The Web TSR has web pages that open in new browser windows. Before you use the Web TSR pages, disable your pop-up blocker or allow pop-ups from the site.

Requirements

Employees of state and allied agencies that do not have NetIDs cannot access the Web TSR. If you cannot use the Web TSR, please use our paper TSR form available through Campus Stores, stock # 11-48-8200.

Accessing and creating a web TSR

  1. Go to the Telecommunications Service Request page.
  2. Use your NetID and NetID password to log in.
  3. The initial page contains links to forms you can use to request services.
    Telecommunications Unit Coordinators who are registered with CITES are authorized to access the forms linked on this page. If you cannot access the forms, contact Service Request Management at 333-1161.
  4. Click on the appropriate form for the service you are ordering.
    A new window opens.
  5. The form will be pre-filled with your default information in your user profile. To change the information for this order, simply type over the data.
  6. Enter the details for your order in the space provided.
  7. Click Continue to submit your order.
  8. A screen summarizing your order appears. Review the information.
  9. Click Submit to place your order or click Edit to modify the information.
  10. Order confirmations:
    • A confirmation screen appears, thanking you for your order.
    • Within two hours, an email message containing the TSR reference number is sent to the address specified in the Web TSR.
    • Within three business days, a Service Request Management representative will email final confirmation of the order to the address specified in the Web TSR. The confirmation will include the TSR reference number, installation date, charges, and any special information regarding the order.

All TSRs submitted after 3 P.M. will not be processed until the next business day.

Updating your user profile

The information contained in your User Profile will be entered automatically in the appropriate field of the TSR forms. If you would like to change this information, follow the instructions below.

  1. Click on the Update User Profile link in the upper-right corner of any page.
  2. A new window containing your current profile information opens. Enter the correct information.
  3. Click Continue.
  4. A screen summarizing your new profile information appears. Review the information for accuracy.
  5. Click Submit to place your order or click Edit to modify the information.
  6. A confirmation screen appears.
  7. Close the window to return to the Web TSR screen.

Remember, you can overwrite any or all of this information on individual TSRs as the details of orders require.

Security

To prevent an unauthorized person from altering or submitting a TSR, you will be automatically logged out if the system does not detect activity, such as typing or clicking on links, for one hour. The TSR will continue to be displayed, but when you attempt to submit the TSR you will be returned to the login screen. Unfortunately, any data you may have entered will be lost, so you will need to create the TSR again.

Support

If you have problems using the Web TSR, call Communications Customer Service at 333-1161 during regular business hours. If the customer service staff is not able to help you immediately, they will refer the problem to a manager who will contact you and help resolve the problem.

Telecommunications Unit Coordinators application or change form

To add or remove a person as a Telecommunications Unit Coordinator, the Telecommunications Unit Coordinators Application/Change Form must be filled out and submitted to Service Request Management.
